MDS, as evidenced by pancytopenia with macrocytosis and Pelger-Huet anomaly on smear.

Pernicious anemia, as evidenced by the particularly high MCV and the hypersegmented neutrophil on the peripheral smear

PNH, suggested by the low haptoglobin.

Myelofibrosis as suggested by the teardrop red cells and nucleated RBCs
